Implant Supported Dentures

What Are Implant-Supported Dentures?
Implant-supported dentures are a permanent tooth replacement solution anchored to titanium implants placed directly in your jawbone. Unlike traditional dentures that rest on the gums, these are secured to the bone itself. The result is a full arch of replacement teeth that looks natural, functions like real teeth, and lasts for decades with proper care.
Traditional dentures sit on top of the gums and gradually cause bone loss as the years pass. The denture that fit well two years ago starts to slip, rock, and cause sore spots. Adhesives help temporarily but never address the real problem. Implant-supported dentures stop that cycle. The implants stimulate your jawbone the way natural tooth roots do, preserving bone density and keeping the shape of your face intact over time.
From a longevity standpoint, the difference is significant. Traditional dentures typically need relining or replacement every five to seven years. Implant-supported dentures, cared for properly, routinely last 15 to 20 years or longer. The implants themselves are often permanent.

Implant Denture Options: All-on-4, All-on-X, and TeethXpress
Dr. Gray offers three implant denture systems at our La Mesa office. During your consultation, he evaluates your bone density, overall oral health, and goals before recommending one. Each system suits different clinical situations, and we will tell you honestly which gives you the best long-term result, not which is most profitable for us.
All-on-4 uses four strategically placed implants to support a full arch of replacement teeth. Two implants sit upright at the front of the jaw, and two are angled toward the back where bone density tends to be higher. That angle often allows patients with some bone loss to get implants without a separate bone grafting procedure, reducing both treatment time and cost. Many patients leave the same day with a temporary arch in place.
All-on-X follows the same principle as All-on-4 but uses more implants, typically six to eight, when additional support is needed or when the bone structure calls for it. It is the preferred approach for patients who want maximum long-term stability or have anatomical factors that make four implants insufficient.
TeethXpress is a same-day implant system designed to place implants and attach a functional set of teeth in a single appointment. If you are currently wearing dentures or have failing teeth that need to come out, TeethXpress may let you walk in with your old teeth and leave with a new set the same day. Our La Mesa office has extensive experience with this system.

Am I a Candidate? What the Implant Process Looks Like
Most adults missing multiple teeth or wearing failing dentures are candidates for some form of implant-supported denture. Good candidates have adequate jawbone density, healthy gum tissue, and no uncontrolled systemic conditions like unmanaged diabetes. If you smoke, you may still qualify, but we will have an honest conversation about how that affects healing and long-term success rates.
Bone loss is the most common concern patients raise, and it is worth addressing directly. The All-on-4 and All-on-X systems were specifically designed to work with reduced bone volume by angling implants into denser areas of the jaw. Many patients who were told elsewhere they were not candidates are successful implant patients today. A 3D CBCT scan is the only reliable way to know for certain, and we offer that scan free for new patients.
Here is what the process looks like from first visit to final result.
Step 1: Consultation and 3D Imaging. Your first visit includes a full oral exam and a complimentary 3D CBCT scan (for new patients). Dr. Gray reviews your bone density and oral health, explains your options, and gives you a clear picture of what treatment involves, how long it takes, and what it costs. No pressure. Just answers.
Step 2: Treatment Planning. We create a precise surgical plan using your scan data. Any teeth that need to come out are scheduled for the same day as implant placement when possible, minimizing the number of appointments required.
Step 3: Implant Placement. Implants are placed under local anesthesia. Sedation dentistry is available for patients who want a more relaxed experience, and it is something our office is particularly known for. Most patients are surprised by how manageable the procedure feels.
Step 4: Healing and Integration. The implants bond with your jawbone over the following months. Depending on the system used, you may wear a temporary denture during this period. With TeethXpress, you leave with functional teeth the same day.
Step 5: Final Restoration. Once the implants have fully integrated, your permanent implant denture is attached. Dr. Gray checks the fit, bite, and aesthetics carefully before you leave. Most patients are back to their normal routine within a few days of surgery.

Frequently Asked Questions
How long do implant-supported dentures last?
With proper care, implant-supported dentures last 15 to 20 years or more. The titanium implants themselves are often permanent. The denture portion may eventually need replacement due to normal wear, but far less frequently than traditional dentures. Regular checkups and consistent oral hygiene are the two biggest factors in long-term success.
Does the implant surgery hurt?
Most patients are surprised by how manageable the procedure is. The surgical area is fully numbed with local anesthesia, and sedation is available for patients who want additional comfort during the appointment. Post-surgical discomfort is typically mild to moderate and controlled with over-the-counter pain medication in the days following. The majority of our patients return to work within a few days.
Can I get same-day implant dentures?
Yes. TeethXpress is a same-day implant system available at our La Mesa office. Failing teeth are removed, implants are placed, and a functional set of teeth is attached in a single appointment. Not every patient is a candidate for same-day placement, but for those who are, it eliminates the extended waiting period associated with traditional implant timelines.
How much do implant-supported dentures cost in La Mesa?
The cost depends on the system used, how many implants are required, and whether any preparatory treatment like extractions is needed. All-on-4 tends to be more cost-effective than All-on-X for patients who qualify, and same-day systems like TeethXpress vary based on case complexity. The most accurate way to get a real number is a proper evaluation. At Espire Dental | Jeff Gray DDS, we provide a full cost breakdown after your free new patient 3D scan consultation so you know exactly what to expect before committing to anything.
Is the free 3D scan offer really free?
Yes. New patients considering implant-supported dentures receive a complimentary CBCT scan at their consultation, a $425 value with no strings attached. The scan gives us the information we need to provide an accurate treatment recommendation, and it gives you real information to make a decision with. Scheduling a consultation does not obligate you to any treatment.
What if I have been told I do not have enough bone for implants?
This is worth revisiting. The All-on-4 and All-on-X systems were specifically designed to work with reduced bone volume by angling implants into areas of denser bone. Many patients who were previously told they were not candidates are successful implant patients today. A 3D CBCT scan is the only way to know for certain, and we offer that scan free for new patients.
How do I clean and care for implant-supported dentures?
Fixed implant dentures are cleaned like natural teeth: brush twice daily, use a water flosser or floss threader to clean between implants, and schedule professional cleanings every six months. Removable implant-retained dentures are taken out and cleaned with a soft brush and denture solution. We walk every patient through their specific cleaning routine before they leave the office.
